Aftercare Planning
At Avalon Malibu, we provide clients with a complete and supportive continuum of care. When an individual enrolls in one of our innovative treatment programs, a multiple-stage plan is put into place for achieving their goal of recovery. In all of our comprehensive treatment programs, a plan for aftercare is built-in, as our intention for clients includes helping them continue down the road to recovery after leaving our facility. Our residential and/or outpatient treatment programs and primary care programs are intensive, so clients are more likely to be successful in maintaining sobriety if they remain actively involved with our recovery community once they return home.
Clients are discharged with a fully formulated plan of action, which outlines the specific goals that the client will continue to work toward. This specialized discharge plan will include referrals to groups and professionals within their own communities. The Avalon Malibu staff will not just throw our clients back into the fire; we will help them through a continuing addiction education program and relapse prevention. We also provide clients with information specific to certain events, such as relapse prevention during the holiday season.
The quality of the aftercare program provided by Avalon Malibu can have a strong influence on a client’s recovery. Our treatment team helps prepare individuals for the days following graduation from the rehab facility. With the right kind of assistance, and therapeutic alcohol and drug support groups, our clients can maintain their hard-won sobriety and continue to build a solid foundation for recovery.

Avalon Malibu specialists recognize that relapse is actually a symptom of addiction, so relapse prevention is included in our aftercare planning. We understand that in order to ensure ongoing recovery, it is crucial that our clients continue to have a support network, long-term guidance, and addiction education after rehab. The primary goal of aftercare planning is to prevent relapse by providing continuous counseling and ongoing support that sets clients up for long-term success. This extra level of accountability helps ensure that each client does not fall back into harmful patterns of substance abuse.
One of the key ways we help clients avoid relapse through our aftercare program is by identifying triggers. The goal of the program at Avalon Malibu is to help our clients work through their issues at work and at home with a proven problem-solving approach. It is important to view this as something equally as important as treatment itself, and as a lifelong commitment. A newly sober person has a greater advantage when they understand the risks and problems involved in recovery.
Included within our aftercare program are transitional services that help clients maintain their sobriety as they re-assimilate into their daily lives. The transitional period after rehab is often the most challenging, so we provide additional support for clients as they learn how to navigate the real world without drugs or alcohol and put their new coping skills into practice. They will receive transitional guidance in approaching family life, career planning, personal responsibilities and more as a sober individual with a new outlook and way of life.
Alumni Support
Recovery is a lifelong journey, and most clients encounter obstacles and challenges along the way. While it’s possible to face them alone, clients that have a supportive community rally around them are much more likely to stay sober, healthy and mentally sound through it all.
That’s why alumni support is an important aspect of our aftercare program for newly discharged clients. Addiction thrives in isolation, but a thriving recovery community can provide individuals with advice, guidance and insight when it’s needed most. In fact, studies show that those who participate in ongoing services after treatment have better long-term outcomes and are less likely to relapse than others. At Avalon Malibu, our alumni support program helps clients maintain their sobriety through support, empowerment and accountability, connecting individuals to like-minded peers who have had similar experiences.
Our services include events for alumni, video chats via Skype, periodic check-ins for reinforcement and more. During alumni meetings, clients can receive advice that’s relevant and applicable to their situation and discuss issues related to life after rehab, such as rebuilding relationships, dealing with cravings, coping with current events and more. They’ll also have access to recovery resources that include educational, professional or volunteer opportunities, or ways to give back to the community by helping others with addiction or mental health problems. Clients can get as much or as little support as they need from our alumni program, but those who stay active and engaged often find it easier to stay sober and handle temptations, triggers or setbacks in a healthy, productive way.
